Healthcare is the largest area of UK public spending, and it buys almost everything: clinical equipment and consumables, medicines, laboratory services, digital systems, estates and facilities, patient transport, and outsourced clinical and care services. But "the NHS" is not one buyer. It is four national health systems with hundreds of purchasing organisations and several national buying bodies, and knowing which one buys what you sell is most of the work.

Most clinical products are not bought through one-off trust tenders. They are bought through frameworks, above all NHS Supply Chain's product frameworks in England, which trusts call off from. For equipment suppliers that means:
Clinical and care services, including community services, diagnostics, patient transport and elective capacity, are commissioned by integrated care boards, trusts and councils. For services involving regulated activities, CQC registration is a pass or fail requirement in England. Social value and workforce questions carry real weight, and many services contracts involve TUPE transfers of existing staff.
Patient transport is a good example of how regional these contracts are: non-emergency patient transport is commissioned area by area, with long contract terms, so the award notice for your area tells you when it will next be competed.
Independent hospital groups are not covered by public procurement rules, so their buying is largely not advertised. The route in is their central procurement team and approved supplier process. Separately, NHS-funded work delivered by independent providers is itself awarded through public processes, which is where independent providers appear in award notices as winners rather than buyers.
Published awards are the most reliable guide to healthcare contract values. Across 411 medical, healthcare and laboratory awards with a published value in the last two years, the typical contract was £100k, and half fell between £42k and £367k. Values range from small consumables call-offs to multi-year service contracts, so compare against awards from similar buyers for similar scope.
NHS trusts and integrated care boards in England publish contracts above £30,000 on Contracts Finder and higher-value work on Find a Tender, running the bid itself through their e-tendering systems. Scotland, Wales and Northern Ireland publish on Public Contracts Scotland, Sell2Wales and eTendersNI, and each runs national health procurement bodies.
Most equipment and consumables are bought through national and regional frameworks rather than trust-by-trust tenders. NHS Supply Chain runs the largest range of product frameworks in England. Devices must meet UK regulatory requirements, including UKCA or CE marking as applicable and MHRA registration, before a buyer can consider them.
Independent hospital groups are private companies and are not bound by public procurement rules, so most of their purchasing is not publicly advertised. Suppliers usually reach them through their central procurement teams or approved supplier lists. Where they deliver NHS-funded care, the NHS contract is awarded publicly, but their own supply chain is not.
If the contract involves providing a regulated activity in England, such as personal care or treatment, the provider must be registered with the Care Quality Commission. Buyers treat it as a pass or fail requirement, so check the activity against CQC's list of regulated activities before bidding.
