Herefordshire Council will be running a Market Engagement event for all businesses interested in delivering home care under a new framework, called Care In Your Home (CIYH) Partnership.
Herefordshire Council has a statutory duty, under the Care Act 2014, to provide home care services for residents with assessed eligible needs, in order to enable them to remain safe and independent in their own homes for longer, reducing or delaying the need for more formal health and social care services.
This engagement follows on from our provider sessions in June and July, as we continue to develop the CIYH model, before decisions are finalised and is an opportunity to:
• Share open feedback on what currently works and what doesn't
• Raise concerns early, while the model is still being shaped
• Influence how the future approach supports providers and the workforce
These engagement sessions will focus on the Discharge to Assess (D2A) element of the new CIYH Partnership, which will involve delivering reablement and enablement to residents after they have left hospital. Reablement / enablement is different to traditional home care, as it is a time limited intervention only (potentially for up to five weeks) with an emphasis on 'doing with, not doing for'.
